Richland County restaurant inspections: critical violations Aug. 19-24

The following restaurant inspections with critical violations were conducted by Richland Public Health between Aug. 19 and Aug. 24:

● Mansfield Area Y, 750 Scholl Road, Mansfield, Aug. 19. Chlorine sanitizing solution at incorrect temperature and/or concentration (critical, corrected during inspection). Observed a bucket of bleach water in the three-compartment sink at 0 ppm.

● Paul Revere Inn, 57 Ohio 97, Lexington, Aug. 19. Handwashing sink is being used for purposes other than handwashing (critical, corrected during inspection). Hand sink in bar area had items in it. Food not properly protected from contamination by separation, packaging and segregation (critical repeat, corrected during inspection). Country-fried steak in Arctic air freezer was uncovered. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected during inspection). Creamer observed in milk cooler was not covered. Equipment food-contact surfaces or utensils are unclean (critical repeat, corrected during inspection). Can opener blade had visible residue. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical, corrected during inspection). Milk in milk cooler open for over 24 hours was not date marked.

● Pretzelmaker (Richland Mall), 2208 Richland Mall, Space #c-8, Mansfield, Aug. 22. Person in charge (PIC) unable to demonstrate knowledge of temperatures and times for safe refrigerated food storage, hot holding, cooling and reheating (critical, corrected during inspection). Education provided for proper cooling times and temperatures. Person in charge did not ensure that employees observed foods as they were received (critical repeat, corrected during inspection). Employee states temperature of TCS food are not being taken upon delivery to ensure they are delivered at required temperatures. At re-inspection, PIC states they have not been taking temperatures of deliveries upon arrival. Education provided during inspection regarding importance of verifying proper temps with deliveries. PIC unable to demonstrate knowledge by having no critical violations (critical repeat, corrected during inspection). Education provided for each critical at time of inspection.

● Foundations For Living, 1451 Lucas Road, Mansfield, Aug. 22. Food not properly protected from contamination by separation, packaging, and segregation (critical, corrected during inspection). Observed raw shell eggs stored on a shelf above packages of sliced cheese inside walk-in cooler #1. PIC moved eggs to the bottom shelf.

● Liberty Nursing Center of Mansfield, 535 Lexington Ave., Mansfield, Aug. 22. PIC not ensuring employees are properly sanitizing equipment and utensils and monitoring of sanitizer parameters (critical repeat, corrected during inspection). Sanitizer strength on warewashing was 0 ppm. PIC is utilizing sanitizer dip in three-compartment sink until unit is serviced. PIC unable to demonstrate knowledge of temperatures and times for safe refrigerated food storage, hot holding, cooling and reheating (critical, corrected during inspection). PIC unable to state times and temperatures for safe cooling. Handout emailed to PIC. PIC did not ensure employees are properly maintaining temperatures of TCS foods during hot and cold storage (critical, corrected during inspection). Ambient temperature of walk-in was 44 degrees and internal temperatures were 43-44 degrees. PIC provided information on temperature monitoring. PIC is to monitor walk-in cooler and ensure it is able to hold internal temperatures of 41 degrees and below. Chlorine sanitizing solution at incorrect temperature and/or concentration (critical repeat, corrected during inspection). Sanitizer in warewashing machine tested at 0 ppm. PIC to use sanitizer dip in three-compartment sink until serviced. Equipment food-contact surfaces or utensils are unclean (critical, corrected during inspection). Ice scoop in dining room storage bin was stored in contact with standing water. TCS foods not being cold held at the proper temperature (critical, corrected during inspection). Walk-in cooler had ambient temperature of 44 degrees. PIC adjusted so unit was at 41 degrees ambient temperature. TCS items with internal temperature of 44 degrees were voluntarily discarded. Ready-to-eat, TCS food not properly discarded when required (critical, corrected during inspection). Thickener with discard date of Aug. 10 was observed in walk-in cooler. PIC voluntarily discarded. Food thermometers are not accurate (critical, corrected during inspection). Probe thermometer at prep table was off by 10 degrees. PIC removed from service.

● V&M Restaurant, 275 E. Main St., Lexington, Aug. 23. Food not properly protected from contamination by separation, packaging, and segregation (critical, corrected during inspection). Observed uncovered containers of chicken inside of the walk-in cooler.

● Kroger Company #N836, 1500 Lexington Ave., Mansfield, Aug. 23. Handwashing sink not accessible (critical repeat, corrected during inspection). Cart was placed in front of hand sink in sushi area. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected during inspection). Sushi observed in contact with sauce packets. PIC removed packets so that they could be sanitized before placing in package.

● Arby's Cam/Rb Inc., 2325 Interstate Circle, Mansfield, Aug. 23. Manual wash solution not being maintained at 110 degrees or above (critical, corrected during inspection). Observed 98 degrees in wash station of three-compartment sink.

● Bellville Duchess Shoppe #1205, 250 S. Main St., Bellville, Aug. 23. Improper washing of hands and arms (critical, corrected during inspection). Worker observed touching contaminated surface of sink faucet with clean hands. Equipment food-contact surfaces or utensils are unclean (critical, corrected during inspection). Worker observed putting unsanitized probe thermometer in food. TCS foods not being cold held at the proper temperature (critical repeat, corrected during inspection). Deli sandwiches were observed cold holding between 45-51 degrees in front display cooler. PIC voluntarily discarded 17 sandwiches. Presence of live insects, rodents, and other pests (critical). Several flying insects were observed in food prep area and contacting food. Please exterminate all pests in food prep area and eliminate any access. Correct by Sept. 6.

● Idea Works Kitchen, 144 W. Third St., Mansfield, Aug. 24. No air gap or approved backflow prevention device on the plumbing system (critical, corrected during inspection). Observed a hose attached to the mop sink faucet without a backflow prevention device. PIC removed hose from sink. Discussed installing an ASSE 1011 backflow preventer if a hose is connected to the mop sink faucet.
