Main St., Annville, PA 17003 4 – 601.11(B-C) Equipment, Food-Contact Surfaces, Nonfood Contact Surfaces, and Utensils Beverage racks within the walk-in cooler are not cleaned at a frequency to preclude the accumulation of mildew and dirt. 4 – 501.114 Mechanical or Manual Warewashing Equipment, Chemical Sanitization The quaternary ammonia concentration in the sanitizing solution of the 3-bay warewash sink was approximately 50-75 ppm, rather than 150-400 ppm as stated on the manufacturer’s use directions. The facility will manually mix the sanitizer to the correct concentration until a service order for the dispenser is completed. 3 – 305.11 Food Storage, Preventing Contamination from the Premises Observed boxes of food stored directly on the floor in the walk-in freezer, rather than 6 inches off of the floor as required. 3 – 101.11 Safe, Unadulterated & Honestly Presented 1 quart-sized container of 1% chocolate milk (date 12/5) and 4 gallon-sized containers of 1% white milk (date 12/11) are offered for sale beyond the expiration dates printed on the containers by the manufacturer. **CORRECTED**

Cumberland St., Lebanon, PA 17042 3 – 305.11 Food Storage, Preventing Contamination from the Premises Observed personal items stored on a shelf above a prep area. 4 – 602.13 Nonfood-Contact Surfaces The fan covers on the evaporator in the walk-in cooler, non-food contact surfaces, are not cleaned at a frequency to preclude accumulation of dirt and soil. 7 – 102.11 Common Name-Working Containers Working containers in the rear prep area, used for storing cleaners taken from bulk supplies, had been marked with the common name of the item, however, the writing had faded to the point of no longer being legible. The issue was noted on October 2021 inspection report for this facility.

Drink ice, scouring pad, and employee drink cups observed in/around the handwash sink (Chester’s side), indicating uses other than handwashing. **CORRECTED** 5 – 205.15 System Maintained in Good Repair Plumbing system not maintained in good repair – observed leaking at the warewashing sink faucet (Chester’s side). 7 – 202.12 Conditions of Use Observed insecticides in the dry storage area (Chester’s side), not labeled by the manufacturer as approved for use in a food facility. **CORRECTED** 2 – 303.11 Prohibition-Jewelry Food employee observed in the food preparation area (Love’s side), wearing a watch on their arm while handling food. 4 – 601.11(B-C) Equipment, Food-Contact Surfaces, Nonfood Contact Surfaces, and Utensils Observed dark biofilm-like residue in the seams of the ice chutes at the soda fountain machine (Love’s side). 6 – 501.16 Drying Mops Mops are not being hung to air dry.

Main St., Palmyra, PA 17078 4 – 301.12 (A-C) Manual Warewashing, Sink Compartment Requirements Food facility does not have a three-compartment sink for manual warewashing. 4 – 702.11 Before Use After Cleaning Observed no chemical sanitizer being used on equipment and utensils after washing and rinsing them, and prior to use. 5 – 205.11 Using a Handwashing Sink-Operation and Maintenance The handwash sink in the kitchen area, which is the only sink in this area, is being used as a food preparation sink as evidenced by loose greens/herbs in the sink.

December 16, 2021 | 365 N. 9th, Lebanon, PA 17046 5 – 203.12 Toilets and Urinals Toilet seat in the employee restroom is broken and on the floor. 6 – 501.111 Controlling Pests Potential rodent harborage areas on the exterior of the building perimeter due to leaves, trash, and other debris along the side of the building. 6 – 501.114 Maintaining Premises, Unnecessary Items, and Litter Rubbish, specifically upholstered furniture, is on the rear porch of the facility.

December 16, 2021 | 640 N. 7th St., Lebanon, PA 17046 5 – 205.11 Using a Handwashing Sink-Operation and Maintenance The handwash sink in the meat department area was blocked by product boxes and not accessible at all times for employee use. Also, there were no paper towels. Corrected. 6 – 501.114 Maintaining Premises, Unnecessary Items, and Litter Frequency of cleaning beneath equipment in the bakery prep area is inadequate as evidenced by excessive debris under equipment. This is of particular concern due to a gap under the door into this area. 7 – 201.11 Separation-Storage Containers of cleaners & chemicals were stored together with food items in the bakery prep area. Corrected. 3 – 307.11 Miscellaneous Sources of Contamination Food utensils were stored on the top ledge of a wall that was not clean. Corrected. 4 – 501.12 Cutting Surfaces Observed deeply scored cutting boards in meat room and hot food prep area not resurfaced or discarded as required. 4 – 602.13 Nonfood-Contact Surfaces The fan covers on the produce walk-in cooler evaporator have an amount of built-up residue. 6 – 202.15 Outer Openings, Protected Exterior door located in the bakery prep area of the food facility has a gap and does not protect against the entry of insects, rodents, and other animals. 4 – 903.11 (A-B)(D) Equipment, Utensils and Linens, Storing Observed clean food equipment in the food prep area, stored wet in a manner that does not allow for draining and/or air drying (wet nesting).

December 17, 2021 | 104 Northside Commons, Palmyra, PA 17078 4 – 602.13 Nonfood-Contact Surfaces Fan guard on the refrigeration unit within the walk-in cooler has an accumulation of static dust. 3 – 501.16(A2)(B) Time/Temperature Control for Safety Food, Cold Holding The meat and cheese side of the refrigerated sandwich preparation cooler was holding time and temperature controlled for safety foods at approximately 50°F, rather than 41°F or below as required. The foods within the cooler were stocked at approximately 7:40 AM according to the person in charge (PIC), who was present during the inspection. The PIC was instructed to relocate the foods to the walk-in cooler to bring the temperatures back down to 41°F or lower to maintain food safety, and a service order was placed for the equipment. **CORRECTED** 3 – 302.12 Food Storage Containers Identified with Common Name of Food Observed food ingredient storage containers, located at the sandwich preparation area, are not labeled with the common name of the food/ingredient. 4 – 601.11(A) Equipment, Food Contact Surfaces, Nonfood Contact Surfaces, and Utensils Tomato slicer was observed to have old food residue and was not clean to sight and touch. 6 – 501.11 Repairing-Premises, Structures, Attachments & Fixtures-Methods Ceiling tiles missing above the walk-in cooler and at the rear of the facility, and need to be replaced.

December 21, 2021 | 1901 Thompson Ave., Jonestown, PA 17038 3 – 302.11 Packaged and Unpackaged Food-Separation, Packaging & Segregation Raw eggs (in shell) stored above ready-to-eat food in the retail cooler. 4 – 602.13 Nonfood-Contact Surfaces Observed webbing along the upper walls and ceiling of the dry storage area. 6 – 501.111 Controlling Pests Observed approximately 12 rodent droppings in the utility closet located at the front of the facility.

December 21, 2021 | 200 Garden Ave., Myerstown, PA 17067 5 – 205.11 Using a Handwashing Sink-Operation and Maintenance Old food residue was observed in the handwash sink located in the main kitchen area, indicating uses other than handwashing. 5 – 205.15 System Maintained in Good Repair Plumbing system not maintained in good repair: – observed leaking at the sink located within the warewashing area. – one of two handwashing sinks within the upstairs women’s restroom does not have hot water being supplied to it. 2 – 301.12 Cleaning Procedure Food Employee did not follow proper handwashing procedure. Soap was not available at the handwashing sinks located at the upstairs bar and in the main kitchen area. Food employee commented that warm water is not used when washing hands. 3 – 302.11 Packaged and Unpackaged Food-Separation, Packaging & Segregation Observed the handle of a plastic bag used to store tortilla shells resting in a pan of refried beans. **CORRECTED** Observed condiment bottles used for dispensing sauces and dressings resting on top of chopped lettuce between uses. **CORRECTED** Raw meat stored above produce (celery) within the walk-in cooler. **CORRECTED** Several foods in the main kitchen area stored open with no covering. 3 – 302.12 Food Storage Containers Identified with Common Name of Food Several food ingredient storage containers, located throughout the food preparation areas, are not labeled with the common name of the food/ingredient. 4 – 501.114 Mechanical or Manual Warewashing Equipment, Chemical Sanitization Both mechanical warewashing machines had chemical cleansers and sanitizers that were fully depleted at the time of inspection. **CORRECTED** 4 – 602.13 Nonfood-Contact Surfaces Fan guards within the walk-in cooler had an accumulation of static dust. 2 – 102.11(A-C1)(C4-16) Person in Charge, Demonstration The Person in Charge does not have adequate knowledge of food safety in this food facility as evidenced by: – failure to prevent potential contamination by hands. – failure to protect food and food-contact surfaces from various sources of contamination. 4 – 601.11(A) Equipment, Food Contact Surfaces, Nonfood Contact Surfaces, and Utensils Blade of a table-mounted can opener, a food contact surface, was observed to have food residue and was not clean to sight and touch. Deflecting arm within the main ice machine was observed to have a bio-film-like residue. 6 – 301.11 Handwashing Cleanser, Availability Soap was not available at the handwash sinks at the upstairs bar and main kitchen area. 6 – 301.12 Hand Drying Provision The handwash sink at the upstairs bar does not have single-use towels. 6 – 501.11 Repairing-Premises, Structures, Attachments & Fixtures-Methods Tile around the floor drain in the warewashing area is damaged and not easily cleanable. Observed pooling of dirty water around the drain/walking area.

December 22, 2021 | 470 Jonestown Rd., Jonestown, PA 17038 4 – 601.11(B-C) Equipment, Food-Contact Surfaces, Nonfood Contact Surfaces, and Utensils Observed pink biofilm-like residue on the outside of the ice chute for the soda fountain machine. 2 – 402.11 Effectiveness-Hair Restraints Food employees observed in the food preparation area, not wearing proper hair restraints, such as nets, hats, or beard covers. 3 – 101.11 Safe, Unadulterated & Honestly Presented Observed commercially packaged cheese stick snacks in the open-air retail cooler that appeared to be partially melted – possibly due to sun exposure. The person in charge was present during the inspection and voluntarily disposed of the product. **CORRECTED**
