Food establishments in Greene County are inspected one to three times per year, depending on the type of food served, the population served, the difficulty level of food preparation, and past history.

Issues found during inspections fall into two categories: priority and non-priority. Priority violations are issues that can have a direct impact on the safety of the food and can lead to a restaurant getting shut down if they persist. Non-priority violations are usually centered on things like sanitation or building maintenance.

Am Pm Food Mart, Phillips 66. 05/19/2022 - Routine Inspection; Result: Active. E. Singh was person in charge at time of inspection. There were a few boxes of food items setting on floor of Indian store (these should be at least 6 inches above the floor). Priority Violations Found: 1. Observed: Chicken and burritos in hot hold cabinet were 117-120 degrees F. These were voluntarily discarded by staff. Nonpriority Violations Found: 3. Observed: Several bags of ice lacked name, address or phone number of where they were packaged. Observed: The inside of ice machine was not clean. Observed: The fan covers and area around them were not clean in beer and main walk-in refrigerator.

Casey's General Store #3013, 1510 N. Glenstone Ave. 05/19/2022 - Routine Inspection; Result: Active. Previous violation was corrected before the inspection. Priority Violations Found: 0. Nonpriority Violations Found: 1. Observed pack of ham being thawed on a counter. Education given corrected on site by staff placing meat in walk-in to thaw properly in 41F or below refrigeration.

Chihuahua Mexican Food, 3747 W. Chestnut Expy. 05/19/2022 - Routine Inspection; Result: Active. Luis was person in charge at time of inspection. Priority Violations Found: 1. Observed: Some cooked chicken, beef and rice lacked "use by" dates. Promptly labeled at my request. Nonpriority Violations Found: 1. Observed: An open container of cooked rice was setting on counter below the paper towel dispenser.

Classic Rock Coffee, 1900 W. Sunset St. 05/19/2022 - Routine Inspection; Result: Active. Priority Violations Found: 1. Observed two small containers of eggs at 48-76F in prep cooler, cooked early this morning. Eggs voluntarily discarded and education provided. Corrected on-site. Nonpriority Violations Found: 0.

Dollar General Store #4527, 2422 W. Division St. 05/20/2022 - Routine Inspection; Result: Active. Elaine was person in charge at time of inspection. Priority Violations Found: 1. Observed: Several containers of detergent were being stored on a shelf above paper and Styrofoam plates and bowls. The detergent was promptly relocated to a bottom shelf. Nonpriority Violations Found: 1. Observed: Several areas of the floor in retail area was not clean.

El Poblano Grill & Cantina, 2629 N. Kansas Expy. 05/20/2022 - Re-Inspection; Result: Permitted with Reinspection. They had bought the faucets, but they have not got them installed yet. I requested that I be contacted once they are installed. Priority Violations Found: 0. Nonpriority Violations Found: 0.

Kum & Go #471, 2953 N. National Ave. 05/25/2022 - Ownership Change; Result: Active. Priority Violations Found: 0. Nonpriority Violations Found: 3. Observed: Some lights over the store aisle and behind front counter found not shielded. Observed: Water damaged ceiling tiles above ice machine and soda dispenser. Observed: Ceiling and electrical conduit in walk-in cooler found not clean. 

Los Cabos, 4109 S. National Ave. 05/19/2022 - Routine Inspection; Result: Permitted with Reinspection. Priority Violations Found: 2. Observed several PHFs being being held above 50F in the walk-in cooler. Observed dish machine not properly sanitizing at the correct concentration for chlorine. Nonpriority Violations Found: 1. Observed employee handle ready-to-eat foods with their bare hands.

Martha's Vineyard, 219 W. Olive St. 05/21/2022 - Routine Inspection; Result: Active. Weston was person in charge at time of inspection. Middle bar sinks are used for doing all the ware washing. One refrigerator was down, but not using any potentially hazardous foods. Priority Violations Found: 0. Nonpriority Violations Found: 3. Observed: Facility was using quaternary ammonia for sanitizing, but did not have any test strips. Observed: Ice scoop being stored in ice bin with handle touching drink ice. Observed: Gender neutral restrooms do not have covered trash cans.

McDonald's, 3510 W. Chestnut Expy. 05/20/2022 - Routine Inspection; Result: Permitted with Reinspection. Discussed hair restraint requirements and testing sanitizer buckets after the addition of the towels. The manager said that they had a work order in for the drive-thru window air curtain. Priority Violations Found: 2. Observed:  Found several ready-to-eat foods not date marked. Observed:  Expired burritos found in reach-in cooler. Nonpriority Violations Found: 2. Observed: Hot foods being held on time only control not marked with discard time. Observed: Single-serve containers not stored covered or inverted. 

Neighbor's Mill Bakery & Cafe, 1435 E. Independence St. 05/24/2022 - Re-Inspection; Result: Active. At the time of reinspection. make table cooling unit was properly cold holding temperature. Priority Violations Found: 0. Nonpriority Violations Found: 0.

Observed buildup accumulation under the dispenser of the soft drink machine. Required to keep NFC surfaces free of accumulation of dust, dirt, residue, and build-up.  Observed hand sink in cooking area leak when in use and have a bucket underneath to collect leaking water. Required to keep plumbing fixtures in good repair.

Papa Murphy's Take N. Bake Pizza, 500 S. Glenstone Ave. 05/24/2022 - Routine Inspection; Result: Active. Priority Violations Found: 0. Nonpriority Violations Found: 1. Observed an employee with a visor hat which is not considered an effective hair restraint. Also multiple employees with facial hair not wearing proper beard restraints. Walmart Supercenter #86 Retail/Bakery, 2825 N. Kansas Expy. 05/24/2022 - Routine Inspection; Result: Permitted with Reinspection. Matt was person in charge at time of inspection. I also checked a complaint during this visit, but did not find anything to validate it. Priority Violations Found: 1. Observed: Watermelon quarters package at facility had a cut date of 5/20/2022 and an "use by" date of 5/29/2022. Nonpriority Violations Found: 1. Observed: The bakery freezer has a moderate amount of ice buildup.

Zan, 311 S. Patton Ave. 05/21/2022 - Routine Inspection; Result: Permitted with Reinspection. John was person in charge at time of inspection. A follow up visit will be made on 5/27/2022 to check on violations. Priority Violations Found: 1. Observed: The dish machine was setup and not washing - rinsing - sanitizing utensils as required. It was pumping in detergent in all three stages and not sanitizing utensils.Nonpriority Violations Found: 3. Observed: Two bar sink were setup, but the quaternary ammonia sanitizer in 3rd compartment had less than 150 ppm quaternary ammonia. Corrected by adding another tablet of sanitizer. Observed: Two of the  women's restrooms were missing covered trashcans. Observed: The mop sink was not clean.
