Greene County restaurant inspections for Dec. 27 - 30

Food establishments in Greene County are inspected one to three times per year, depending on the type of food served, the population served, the difficulty level of food preparation, and past history.

Issues found during inspections fall into two categories: priority and non-priority. Priority violations are issues that can have a direct impact on the safety of the food and can lead to a restaurant getting shut down if they persist. Non-priority violations are usually centered on things like sanitation or building maintenance.

Here is information from the Springfield-Greene County Health Department regarding restaurant inspections over the past seven days:

Bawi, 4121 S. National Ave. 12/29/2021 - Routine Inspection; Result: Active. Priority Violations Found: 0. Nonpriority Violations Found: 2. Observed two hand sinks on cook line without paper towels. Corrected by re-stocking. Ensure hand sinks have paper towels and soap prior to beginning prep for the day. Observed grease build up under grill on low boy and on side of fryer.

Cactus Ridge Cafe Llc, 215 S. Washington St., Walnut Grove. 12/28/2021 - Routine Inspection; Result: Active. Melody was person in charge at time of inspection. One light fixture over prep refrigerator was damaged and hanging. Priority Violations Found: 1. Observed: There was a tray of sausage in oven that was 100 degrees F. Promptly reheated to over 165 degrees F. Nonpriority Violations Found: 3. Observed: The facility's thin probed thermometer was not operational. Observed: There was a chub of raw ground beef being stored above canned soda in refrigerator. The ground beef was promptly relocated to bottom shelf of another refrigerator. Observed: There was a plastic bucket of soapy water setting in hand sink. Promptly removed at my request.

Costco Wholesale #1486 Bakery/Retail, 279 N. Eastgate Ave. 12/28/2021 - Routine Inspection; Result: Active. No violations observed at the time of inspection. Priority Violations Found: 0. Nonpriority Violations Found: 0. 

Costco Wholesale #1486 Deli, 279 N. Eastgate Ave. 12/28/2021 - Routine Inspection; Result: Active. Priority Violations Found: 1. Observed employee taking temperature of chicken pulled out of hot hold unit without sanitizing blast chiller thermometer probe. One piece of chicken voluntarily discarded. Manager provided education and employee sanitized the probe. Corrected on-site. Nonpriority Violations Found: 0.

Costco Wholesale #1486 Food Court, 279 N. Eastgate Ave. 12/28/2021 - Routine Inspection; Result: Active. No violations observed at the time of inspection. Priority Violations Found: 0.  Nonpriority Violations Found: 0.

Costco Wholesale #1486 Meat, 279 N. Eastgate Ave. 12/28/2021 - Routine Inspection; Result: Active. Priority Violations Found: 0. Nonpriority Violations Found: 0.

Freddy's Frozen Custard & Steakburgers, 2305 N. Glenstone Ave. 12/27/2021 - Complaint; Result: Suspend, Public. On site for complaint inspection. Facility had handwashing water with a maximum water temperature of 55F. Facility closed due to Imminent health hazard: no hot water. Food establishments are required to have minimum hot water temperature of 100F. Facility is closed until approved to open by Springfield Greene County Health Department. Priority Violations Found: 1. Observed employee put on gloves without first washing hands. Corrected by having employee wash hands first. Employees must wash hands after handling raw meat, before donning gloves, and anytime contamination occurs. Nonpriority Violations Found: 1. Observed water with maximum temperature of 55F. Provide water of at least 100F to all hand washing sinks.

Freddy's Frozen Custard & Steakburgers, 2305 N. Glenstone Ave. 12/28/2021 - Suspended Permit Reinspection; Result: Approved. Arrived on site after call from district manager requesting to be reopened. Upon arrival observed facility operating without prior health department approval. Verified water could reach 100F at all sinks. Facility is approved to reopen. Imminent Health Hazard: No hot water, has been corrected. Priority Violations Found: 0. Nonpriority Violations Found: 0. 

Galloway Station, 1281 B E. Republic Road. 12/28/2021 - Routine Inspection; Result: Permitted with Reinspection. Priority Violations Found: 1. Observed:  Foods held at temperatures above 41F in small prep cooler. Nonpriority Violations Found: 0.

Hilton Garden Inn, Pavilion Grill. 12/27/2021 - Routine Inspection; Result: Permitted with Reinspection. Priority Violations Found: 2. Observed several PHFs being held above 44F inside the make table. Items discarded. A reinspection will be performed. Observed several ready-to-eat PHFs not properly date marked with the 7 day discard date. Nonpriority Violations Found: 0. 

Hinode Japanese Steakhouse, 4301 S. National Ave. 12/27/2021 - Routine Inspection; Result: Permitted with Reinspection. Priority Violations Found: 4. Observed celery and lemons touching shell eggs in walk-in cooler; observed shrimp in bottom of reach-in cooler below beef. Shrimp juice was running over the container into the bottom of the unit. Corrected, shrimp moved to larger container and placed above beef, unit sanitized. Celery and lemons moved to alternate area in walk-in cooler. Education provided. Manager agrees to retrain employees. Corrected on-site. Observed reach-in cooler holding fish, scallops and shrimp at 43-47f. Items voluntarily discarded. Manager agrees not to store PHFs in unit until repaired. Observed seafood sauce at 50F, above storage line limit in wait staff prep cooler. Individual seafood sauce containers voluntarily discarded. Manager agrees to educate staff on storage. Corrected on-site. Observed several bowls with food crumbs in on wait staff line. Observed dish racks with build up of black debris/grease. Bowls sent to dish room for washing immediately. Manager agrees to replace or deep clean dish racks. Nonpriority Violations Found: 2. Observed build up of food debris and or grease on vent hoods, inside coolers, outside of equipment such as grills, etc. Deep cleaning needs to occur and then cleaning frequency of equipment increased. Observed wall board in dish room beneath dish machine and on wall behind dish machine separating from the walls. Reseal or replace, the wall board should be sealed tightly. 

Hinode Japanese Steakhouse, 4301 S. National Ave. 12/29/2021 - Re-Inspection; Result: Permitted with Reinspection. Core item of back dish room wall has been corrected. Follow up will occur by 1/28/2021 on the wall beneath the dish machine and the 3-door reach-in cooler. Priority item of dish racks has been corrected. there were several new racks available for ware washing. Priority Violations Found: 1. Observed cooler holding at 45-48F. No food is being stored in unit at this time; a part has been ordered for the unit. Nonpriority Violations Found: 3. Observed build up of grease on vent hood filters (manager stated cleaning company is scheduled in January of 2022) and on sides of fryer. Other non-food contact surfaces of equipment and walls have been cleaned. Correct by 1/28/2022. Observed shrimp in walk-in cooler stored next to raw chicken, sides of containers touching. Observed shrimp under beef in reach-in cooler. Repeat items. Store raw animal food in correct order and by separating. Education provided. Corrected on-site. Future repeat violations relating to cross contamination will result in a risk control plan being issued. Observed wall paneling beneath dish matching separating from wall. Back wall of dish machine was in process of being repaired upon arrival, a new panel board was in place and the area was in the process of being sealed. The repair man stated the area under the dish machine will be repaired as well. Correct by 1/28/2022.

Jamaican Patty Co., 3439 S. Campbell Ave. 12/27/2021 - Routine Inspection; Result: Active. No priority violations noted. Priority Violations Found: 0. Nonpriority Violations Found: 0. 

Kung Fu Taco, 3354 S. National Ave. 12/29/2021 - Routine Inspection; Result: Active. Priority Violations Found: 0. Nonpriority Violations Found: 2. Observed food debris on floor of the back kitchen. Needed to be cleaned before next routine inspection. Observed 3-vat sink that leaks, awaiting repair.

La Quinta Inn & Suites, 2445 N. Airport Plaza Ave. 12/30/2021 - Routine Inspection; Result: Active. Priority Violations Found: 2. Observed 11 dented cans stored in kitchen, not separated from stock. Education given, cans were removed from service and stored in a designated area, away from cans to be used for service. Corrected on site. Observed can opener not clean. Instruction given and can opener was removed to be cleaned, rinsed and sanitized. Corrected on site. Nonpriority Violations Found: 1. Observed hand wash sink in kitchen had a maximum temperature of 91 degrees. Water temperature should be a minimum of 100 degrees. Repair prior to next routine inspection. 

Missouri Mike's Bbq & More, 1724 E. Battlefield Rd. 12/28/2021 - Routine Inspection; Result: Active. Priority Violations Found: 1. Observed three chemical spray bottles near single-service items or food contact services throughout the establishment. Education given. Corrected on site by moving chemical spray bottles away and below food, equipment, single-service, single-use articles. Nonpriority Violations Found: 1. Back door of kitchen has a gap at bottom. Repair/replace by next routine inspection.

Red's Giant Hamburg Inc, 2301 W. Sunshine St. 12/29/2021 - Re-Inspection; Result: Active. Tish was person in charge at time of reinspection. All violations cited on 12-22-2021 inspection have been corrected. Priority Violations Found: 0. Nonpriority Violations Found: 0. 

Scramblers, 1131 E. Republic St. 12/23/2021 - Routine Inspection; Result: Active. Priority Violations Found: 1. Observed:  No sanitizer in 3-compartment sink. Nonpriority Violations Found: 0. 

Taco Bell #001695, 2933 S. National Ave. 12/27/2021 - Routine Inspection; Result: Active. Priority Violations Found: 1. Observed: two sanitizer buckets near front counter did not have concentration of sanitizer as required by manufacturer's specification. Reading less than 150 ppm. Required: sanitizing solution should be 150-400ppm. Corrected on site. Nonpriority Violations Found: 1. Observed several boxes of food products being stored on the floor of the walk-in freezer. Required: food shall be protected from contamination by storing the food at least 6 inches above the floor. Correct by next inspection. 

Test 6/25/2021, 220 E. Central St. 12/28/2021 - Routine Inspection; Result: Permitted with Reinspection. Priority Violations Found: 0. Nonpriority Violations Found: 0. 

Texas Roadhouse, 255 E. Monastery St. 12/27/2021 - Routine Inspection; Result: Permitted with Reinspection. Discussed requirement for soap at hand sinks and employee drink requirements. Priority Violations Found: 1. Observed:  Two spray bottles of cleaners not labeled. Nonpriority Violations Found: 2. Observed:  Drain on dish machine not draining properly, something is stuck inside.  Allowing water to spill onto dish area floor going into floor drain. Observed:  Several food prep employees with no required beard restraints.

Wendy's #01, 225 W. Sunshine St. 12/27/2021 - Routine Inspection; Result: Active. Priority Violations Found: 0. Nonpriority Violations Found: 0. 

Wendy's #33, 3558 S. Campbell Ave. 12/23/2021 - Routine Inspection; Result: Active. No violations noted. Priority Violations Found: 0. Nonpriority Violations Found: 0.
