Vigo County, Indiana (WTHI)-The following are food inspections conducted by the Vigo County Department of Health between November 1 and November 6, 2021. 

Sam's Southern Eatery, 920 S 3rd St- (5 severe, 8 non-serious) opened mayonnaise container found at room temperature. Hush puppy and French fries were found on the 62F countertop (discarded). Observe the green beans in the hand wash sink. Observe the flour and dried bread crumbs in the sink after observation. The kitchen hand wash sink was found to be dirty. The soda nozzle was found to be dirty. Green beans are cooked on a flat-top grill in original cans.

Hacienda Coyote Grill, 2433 S 3rd St- (4 serious, 2 non-serious) A dirty spoon and a pot of water were found in the sink. Disinfectant is not measured in the dishwasher. A pile of pink debris was observed on the baking soda gun and bracket. Observe dead and live insects in the kitchen and bar area.

La Isla Mexican Restaurant, 1480 Ft Harrison Rd- (3 critical, 2 non-critical) employees have no certified food managers. It was found that the hand wash sink on the grill/production line was clogged. Several items in the walk-in cooler found at 47-51F should be 41F or lower.

Country Mark, 9950 S US Hwy 41- (2 serious and 2 non-serious) The milk in the 55F refrigerator should be 41F or lower. The hot items in the incubator do not indicate when to discard them.

Pizza Gallery, 6710 Wabash Ave- (2 severe, 1 non-serious) soda water nozzles and ice chute black debris was observed. Prepare the table cheese container with blue tape to cover the cracked/broken edges.

Speedway #5173, 3388 S US Hwy 41- (1 serious, 2 non-serious) soda nozzles are dirty.

Applebee's Neighborhood Grill, 2506 S 3rd St- (1 serious, 2 non-serious) No disinfectant was measured in disinfectant buckets throughout the kitchen.

MCL Cafeteria, 3 Meadows Lane-(1 severe, 2 non-serious) Cockroaches were observed on the wall around the pipe and the dish area on top of the dishwasher.

Royal Mandarin Express, 43 Southland Ctr- (1 severe, 1 non-serious) soda nozzles found black debris.

TeePee Tavern, 1651 S 25th St- (1 serious, 1 non-serious) Small insects around the soda gun and beer tap line were observed behind the bar.

Seelymart, 9681 E US Hwy 40-(1 Critical) Debris was observed in the ice maker.

Sycamore Manor, 222 S 25th St- (1 Critical) Ice Maker observed pink fragments.

Panera Bread Bakery/Café #1088, 5653 S US Hwy 41-(1 Critical) Burnt crumbs and greasy film were found on several baking trays and bread trays.

Wing Stop, 2828 S 3rd St- (1 Severe) Food residue was found in the employee's hand wash basin.

Rally's, 3492 S US Hwy 41-(1 Critical) Several cloth buckets without disinfectant were found.

Arby's, 3670 S US Hwy 41-(1 Critical) Disinfectant is not measured in mechanical dishwashers.

Speedway #7717, 2445 Hulman St- (1 Severe) Pink fragments were observed on the ice trough of the fountain machine and the ice machine in the background.

Outback Steakhouse, 3700 S US Hwy 41-(1 Critical) A half-pan used for roasting potatoes, found to have burnt crumbs.
